The executioner should spawn when first boss is killed and give a 55 min timer before the prisoners are killed. However it doesn't spawn at all in heroic difficulty so you cannot obtain he quest item.

yes sure this quest did used to work but since the instance got rescripted by trinity it does not,

have found the area triggers for this, however cannot fix this, as it has to be implemented into instance script as it should display timers for the execution of the npcs which should start after you kill the warlock boss and enter passage behind her.
