I think it is a good idea to require max level retail character - ensures that the IP owner has actually gotten a decent cut.

"After you hit the submit button, there will be a green notification at the top right portion of the page that says "Your request has been submitted" or something of that nature."

So i submitted my request 2 times and both there wasnt any green notification so did they removed it or im doing something wrong ?

If there is no confirmation, the request was not submitted. Check if you filled all the fields and specially if you checked the "agree" check box.

Be doubly sure you have a Game Account (not Forum Account) and that you have selected it in the Application Form. Create a level 1 character on that account.

I am having trouble with the second screenshot (password field). Since my account is inactive on retail, I am no longer able to show the character list on the Blizzard home page website. The best I can manage is showing the character list in the Technical Support (since that is the only place you can post without an active sub.)

Is this acceptable for the transfer application?

As long as we can see a list of your characters, including the one to be transferred, it should be fine.

We do not use in game pictures. Just submit the remaining screenshots and they will be considered.

We offer 1 template and one copy character, if you are willing to forfeit the copy character. I can add the additional template transfer.

Ok, the transfer does not work. Block because of the last one. Can't choose copy or template (greyed out).
