The battered hilt!

My problem is when transitioning between the 2 stages of the chain.
I have completed the quest "Return To Caladis Brightspear" including watching the cinematic.
After wards Caladis Brightspear should give the next quest in line of the queue "Reforging The Sword".
This step didn't happend so i don't have any new quest / object in the inventory.

the restart / server crashed - fixed the problem, i have the new quest

the next problem in chain is the quest: "tempering the blade"
I have thrown the blade in the crucible (forge of souls) as intructed by the quest, yet nothing happen neither on normal, or heroic.
The quest doesn't say if the bosses in Fos must be killed before that - so i don't know if that is the problem. Also the quest is not added in quest completer.

but the npc where i should turn the quest is not spawned.
any chance for spawning him?

Server reset / crashed - i could turn quest now and get new stage
