The cause for Headless Horseman bugs

I think I've identified the cause for Headless Horseman bugs.
It appears that bugs occur whenever the total health of the head is brought down to below what it should spawn with in the next transition phase, either through big damage single hits or through dots applied to the head.
Every time the encounter bugged, the head spawned with an incorrect % of health remaining in one of the transition phases (it should spawn with 100% remaining in the first transition, 66% health remaining in the 2nd transition, and finally 33% health in the final phase (I don't know whether that's blizzlike, but I can clearly see this was the intention of the TW script for the encounter)).
Keep observing and you will notice the same.
I'm not stating this based on 1-2 cases. Over the past 2 years I've had around a 100 bugs happen.
I'm also guessing that the head's health % is the trigger for the next normal Headless Horseman phase, which is exactly why it's so important that the health % of the head is correct every time it spawns, or else the Horseman will permanently remain in invulnerable stage.
Another explanation could also be that the bugs are occuring because transitions were scripted to trigger on a certain % of head's health taken out in any given transition phase instead of being triggered by when its health reaches certain % (I hope you understand the difference). This would explain why the bug only occurs after the final transition phase since the overkill in the first two transition phases means there won't be enough health remaining for this kind of trigger to activate.
